Co-payments and deductibles are due at the time of service. We accept major credit cards, checks and cash.
We accept most major PPO health insurance plans such as those listed below.
It is the patient’s ultimate responsibility to ensure your insurance is in-network. If you are in doubt, please contact your insurance company directly. This list is not a complete list of insurance companies. This list is subject to change at anytime. To ensure you are in-network with our office, please check before making an appointment.
Knowing whether we are in network with an insurance carrier is the responsibility of the patient. Our office will, as a courtsey, do its best to verify your insurance eligibility, however, given the plethora of sub-plans under each insurance carrier, the ultimately responsibility is the patient's. Any balance due as a result of insurance ineligibility or out of network plan is the financial responsibility of the patient.
All cancellation must be made greater than 24-business hours before the scheduled appointment, or a cancellation fee will be applied to the patient. This fee will total $50.00 for all regular appointments, and $100.00 for all cosmetic and surgical procedures.
We adhere to a strict 15-MINUTE grace period for appointment arrivals, or visits will need to be rescheduled.
Parking Validation
We are unable to validate for parking at our facility.
Pets and emotional support animals are prohibited from accompanying patients to appointments in order to adhere with our medical facility policies. Registered Service Animals are exempt to comply with state laws, including any other exemptions as required by law.
Pure Dermatology & Skin Surgery Center requires that all patients looking to utilize their medical insurance to also have a credit card on file. For any outstanding balances, patients will be sent an invoice, and if unpaid, the total balance will be charged to the credit card on file.
